Plan details
- Waiting period: none for any service
- Deductible: $50/person in Year 1, $25 in Year 2, $0 from Year 3 on (three per family; waived for preventive)
- Annual maximum: $1,500 per person
- Dental accidental injury: covered injuries paid at 100%
- Maximum carryover: build up to an extra $1,000 toward your annual maximum
